How is Xanthelasma Surgery Performed?

What is Xanthelasma?

Xanthelasma is a skin condition characterized by yellowish fat deposits that form on the eyelids. It is generally harmless but can be aesthetically disturbing. This condition may be associated with high cholesterol or lipid metabolism disorders, but in some cases, it can develop without any underlying health problems.
Although xanthelasma is completely harmless, many patients may want to remove these formations due to cosmetic and aesthetic concerns. Xanthelasma can be treated using various techniques such as surgical methods, laser treatments, and chemical peeling.

What is Xanthelasma Surgery?

Xanthelasma surgery is a surgical procedure performed to remove fat deposits from the eyelids. Although it is a minor operation, it must be performed by an experienced ophthalmologist, as there is a risk of scarring if the correct techniques are not applied.

How is Xanthelasma Surgery Performed?

1. Patient Evaluation and Preparation

  • First, the patient undergoes a detailed examination, and the size and extent of the xanthelasma are assessed.
  • Blood tests are performed to check cholesterol levels, as xanthelasma is sometimes associated with high cholesterol.
  • The patient’s skin type and healing process are evaluated to minimize the risk of scarring after surgery.

2. Anesthesia Application

  • Xanthelasma surgery is performed under local anesthesia, meaning the patient is awake but feels no pain.
  • After anesthesia is applied, the area is completely numbed.

3. Surgical Intervention

  • The fat deposit on the eyelid is carefully removed using specialized microsurgical techniques.
  • The fat plaques are cleaned without damaging the surrounding tissues.
  • No excess skin tissue is removed during surgery because the eyelid is a sensitive area, and excessive tissue removal could cause eyelid sagging.

4. Stitches and Dressing

  • If a large incision is made during the procedure, fine cosmetic stitches may be used. However, stitches are usually not necessary in most xanthelasma cases.
  • After the procedure, the area is covered with an antibiotic ointment, and the patient is given information about the recovery process.

Recovery Process After Xanthelasma Surgery

✅ There may be slight redness and swelling in the first few days, but this usually resolves on its own.
✅ If stitches are used, they are typically removed within 5-7 days.
✅ Although the risk of scarring after surgery is low, there is no guarantee of completely scar-free healing.
✅ Since the eye area is sensitive, heavy makeup and irritating products should be avoided during the healing process.
✅ Patients can generally return to their daily activities within a few days.

Does Xanthelasma Surgery Recur?

Xanthelasma surgery can offer a permanent solution, but in some cases, it may recur. Especially if the patient has high cholesterol or if lipid metabolism disorders continue, new xanthelasma formations may occur.
To prevent recurrence of xanthelasma: ✔ Regular cholesterol checks should be done.
✔ A healthy diet should be followed.
✔ Smoking and alcohol consumption should be limited.
✔ The eye area should be regularly moisturized and protected.

Can Xanthelasma Surgery Be Treated with Alternative Methods?

Apart from surgical treatment, different methods can also be used to address xanthelasma: 1️⃣ Laser Treatment for Xanthelasma

  • The laser destroys the xanthelasma tissue by burning it.
  • It may leave less scarring compared to surgery but may not be suitable for every case.

2️⃣ Chemical Peeling for Xanthelasma

  • Xanthelasma can be removed by peeling the skin surface with chemical solutions such as TCA (Trichloroacetic Acid).
  • It is effective for mild cases but may be insufficient for deeper lesions.

3️⃣ Radiofrequency Treatment for Xanthelasma

  • Xanthelasma tissue is vaporized and destroyed using radiofrequency waves.
  • It is generally preferred for small xanthelasma formations.

Is Xanthelasma Surgery Risky?

Xanthelasma surgery is generally safe because it is a minor surgical procedure; however, like any surgery, it carries some risks: ⚠ There may be mild swelling and redness.
⚠ In very rare cases, scarring may occur on the eyelid.
⚠ The possibility of recurrence cannot be completely eliminated.
When performed by an experienced surgeon, these risks are minimized, and the procedure is typically successful.

How Much Does Xanthelasma Surgery Cost?

Xanthelasma surgery costs can vary depending on: The clinic’s location,
The patient’s condition,
The method used.
Since SGK (Social Security Institution) or private health insurance usually considers xanthelasma a cosmetic problem, they may not cover the procedure. However, xanthelasma that affects the visual field or causes discomfort may sometimes be covered by insurance.
